Drive-Thru Flu Clinics - Hosted by Farmington Valley Health District & Farmington Valley VNA

The Farmington Valley Health District (FVHD) in collaboration with the Farmington Valley Visiting Nurses’ Association (FV VNA) is hosting two separate drive-thru influenza vaccination clinics this fall. Both agencies are actively engaged in COVID-19 response activities and wish to ensure the residents of the Farmington Valley are prepared this fall to safely receive their annual flu shot.

A drive-thru clinic provides a safe alternative to public clinics and ensures adequate social distancing.

Interested residents ages 9 years old and up are eligible to receive the flu shot.

The dates of the two clinics are:

  1. Wednesday September 30, 2020 from 1-5pm at the Avon DPW facility on 11 Arch Road, Avon
  2. Wednesday October 7, 2020 from 1-5pm at 251 Weatogue Fire Station on Hopmeadow Road, Simsbury

Pre-registration is required for all residents who wish to be vaccinated. A link to the pre-registration survey can be located on both the FVHD and FV VNA websites. Additionally, once you complete the pre-registration form, you MUST also complete the online medical screening questionnaire. Once completed, print this page and bring it with you. Pre-registration will close approximately two weeks before each clinic. No walk-ins (or drive-ins) will be allowed. Vaccine is limited to 200 persons per clinic.

Bring the following items with you on vaccination day:

  1. License (as applicable) and insurance card for each person
  2. Printed copy of the medical questionnaire
  3. Wear loose fitting, short-sleeve shirt
  4. All residents MUST wear Face Mask
